First and reverse are ok and third is ok so the problem is in the intermediate servo. Try adjusting the band. If it hasn't got any shift kit in it (unless it is a B&M kit) then use the factory setting:
Loosen the lock nut (3/4") and then tighten the adjusting screw to 10 foot pounds. Back the adjusting screw out 1 1/2 turns. Hold the adjusting screw and tighten the lock nut.

If you have a kit in the valve body then follow the instructions in the kit to adjust your band.

If the band adjustment doesn't solve your problems then you can try removing the servo and resealing the servo. If that doesn't work or the band does not adjust then you will have to pull the transmission and replace the band - you might as well rebuild it at that point.

On the left (driver's) side near the front of the case (behind the bell housing) is the adjustment. Just loosen the 3/4" lock nut enough to turn the adjusting screw (5/16 square). Hold the lock nut while you turn in the adjusting screw with a torque wrench to 10 foot pounds. Then back out the adjusting screw the 1 1/2 turns while still holding the lock nut. Then hold the adjusting screw while you tighten the lock nut.

Changing the fluid and cleaning the filter (it is a screen) in clean solvent is a good idea but it is probably unrelated to your problem. If you do change the fluid be sure to use only type "F" automatic transmission fluid. There is no substitute for the type "F" but you can use any brand of type "F" that is available.

Let us know how it turns out. - - Oh, and if the band won't tighten then the band might be broken or completely worn out. In that case the transmission will have to be rebuilt. If it does adjust it might be worth installing a TransGo 40-2 shift kit in the valve body. The kit will help the transmission last as long as possible but it will not repair a broken transmission. It will do no good to install it without the C4 operating properly to begin with.
