Usually caused by too much boost from the turbo.

Possibly a bad blow off valve, or the wastegate got stuck(if he is running a turbo with a wastegate) or he just had too much boost and the engine didnt like it at the moment..

6.0l diesels are good for that at anything over 30psi boost pressure.

How much boost are you running and is the motor relatively stock?

Too much cylinder pressure for the stock tty head bolts ls motors with stock bolts generally dont like to see the pressures that are generated with "higher" boost levels

25 psi when it lifted the heads not uncommon with stock parts at 9.5.1 compression it wasn"t a tune or detonation / air temp issue just a matter of stock tty bolts not handling the clamp load arp studs and mls gaskets are in the future along with some 6 liter heads

I was aware of the risk you can run 20psi on these motors all day long on stock parts ive been doing a little late night racing and do not take a chance on getting outran ;)

The little 350.00 motor that could

I'd say you dodged the bullet on that one. The fire ring is toast.........
That is an odd spot........from the picture it looks like wall is totally clean under the spot..........and not so clean above. What did the plugs look like?
I see studs in your future and maybe a thicker gasket. Are those gaskets .040 or .060 compressed?

Plugs look great nothing out of line really, talked to a guy about a aluminum 5.3 block tonight would be nice to take that 100 lbs off the nose.i think maybe that cylinder had some water sit in it at one time (i bought it from a salvage yard) debating pulling it and freshining it or slap some gaskets and studs in it

No idea on the gasket thickness there oem gm gaskets this is the first time ive had the heads off.
